- copy of drivers license
- copy of builder's or architect license (if applicable)
- $25 registration fee for 2 years or $15 renewal fee for 2 years; $5 replacement fee for lost cards
Please Note:
Providing your company meets the requirements as stated in our
registration policy, your registration card will be issued within
10-14 days after receipt and approval of this completed form and
all support information.
FOR COMPANIES ALREADY REGISTERED WITH MDC:
AN APPLICATION FORM, BUSINESS CARD AND DRIVER'S LICENSE MUST BE SUBMITTED FOR EACH STAFF MEMBER
THAT WISHES TO UTILIZE MICHIGAN DESIGN CENTER AND IT’S SERVICES.
IN ADDITION, WE MUST RECEIVE A LETTER, ON COMPANY LETTERHEAD,
VERIFYING THE EMPLOYMENT OF EACH APPLICANT AND STATING THAT EACH
IS ALLOWED TO BE ADDED TO THE COMPANY REGISTRATION. |
